Day of Service 2011 Group Creation Form

Please use the form below to create a new group to participate in the 2011 Day of Service. Registration for the Day of Service will begin in August. By creating a group, participants will have the option of joining your group and serving together at the Day of Service.

Please note: If you are participating in the Day of Service, you will still need to complete the Day of Service registration form. Please allow 24 hours for the group you have created to appear as a service group option (be sure to select this group if you want to be added as a member).

Please create a group name below. Group names should be descriptive (i.e. Orientation Group 34, Fedigan 3rd Floor, ACS 11:30 Dr. Smith) and be easy for your team members to remember. Please include additional information in the description field.
